Completed form must be returned no later than January 15, 2011 to be included in the Olympics.

Participants must be dressed for the Olympics just as they would be dressed for a Hockey game. We must insist on this for safety and fairness. Please note! There are a maximum of 17 positions to be filled per team, unless your team has more than 17 members. Our intent is to have room for each player to be in one event.
